It's a nonsense term. For some engines people use it to refer to engines with displacement over a certain amount, other engines people use it to refer to the physical size of the engine, usually the deck height compared to the 'small block' engines by the same manufacturer.

It is actually a factory specification. There are often 2 casts for V8 engine blocks: big and small. What makes it a big or small block is what it was originally cast from, not what it is bored and stroked to.

And the 403 Olds is a small block.

You're wrong on a couple of counts.
The Pontiac V8 is not a big block or a small block
The Iron duke shares bore and stroke with the V8 but isn't developed from it
Pontiac did produce a 4 based on the 8 but it was called the trophy 4
