Gravewalker (PF) in our group used a skull instead of the doll. Creepy as fuck crazy homeless elf in Westcrown. Her patron convinced her to murder a girl (npc) who had grown into a relationship with her (convincing her to use the girl's skull as her new familiar).

Stealing youth, changing shape, riding brooms, chatting with satan, eating people. Fun witch activities.

A really powerful old witch doesn't need a disguise, she just simply makes a character(s) completely fall in love with her, yandere mode,
Which is much more scary for the players, and more fun to play as well.
