How was Zossimov not real?

No one in the book acknowledges him apart from Raskolnikov, also he is absent whenever Raskolnikov is no longer 'feverish'

Here's a thinker: He is the only that actually exists and the fever delirium makes Raskolnikaf imagine his surrounding and his well-being.

He talks to Razumikhin and raskolnikov's mom

Both those characters interact with other characters though. Even when raskolnikov isn't around.

This is a common misunderstanding, we, the readers, never actually 'see' them interacting with other characters.
All information we get is either hearsay or imagined

Pyotr Petrovich seeks out Zossimov's approval when he first clashes with Razumikhin.

Zossimov, Nastasya and Razumikhin all discuss the murder when Raskolnikov is unconscious.

That wouldn't add anything to the story though.
It wouldn't fit with the rest of Dostoevsky's works either.

Also, razumikhin brings zossimov to raskolnikovs mom from the party when raskolnikov was delirious on his couch. Bringing the doctor directly to here was the only way she would leave raskolnikovs apartment.
