They're called 6* mega rank-up tickets. Sometimes they're sold alone for 2,750 crystals or in a tiered sale for 2,600 crystals. There's also the various packages in the store which cost real money. If you think somehow you can get them for free then you're going to be disappointed.

If I upgrade to 4, then the 3rd and 4th item is randomly generated? If I then upgrade that to 5, the 3rd and 4th are randomly generated again?

In that case, I'd be best to keep any card that has the stats Im seeking - like a card with Recovery Rate, Skill Cooldown and Defense Penetration shouldnt be re-rolled?
