You acquire a kidney (left or right, doesn't matter), either by buying from a trader or harvesting from a prisoner or colonist. You then install the kidney in place of the lost one.
Surgery is still extremely RNG, so ensure that you have glitterwood meds, a hospital bed, and a decent doctor or you'll risk damaging the patient and losing the kidney.

If a surgery fails it'll add lots of wounds on the patient's body. This is only really a problem if the pawn was wounded to begin with, in which case you risk losing limbs. Technically there is a chance that you kill them outright from full health, or you damage their organs permanently, but it's pretty slight.
In my opinion, leaving a colonist with just one kidney is the bigger risk; your blood filtration is severely hampered, and if the pawn gets hit with a disease he'll have a really hard time accruing enough immunity to survive.

the thing is with both it's +40% pain which results in them always near major mental breaks from pain, 170% tiredness which means they sleep almost double the other colonists, and 200% hunger which means they eat twice as much as other colonists.

They sleep longer, but they have increased manipulation to counteract that. And they move faster, so they spend less time in transit.
And if they get hurt, they get back up faster due to increased blood pumping. You're just not very good at the game.

You have to remember that "Alpha" means basically nothing nowadays and it's all about how long the developers want to support the game.
The game could get to A18 and then he could release it as "1.0" or some shit and people would accept it. Steam Early Access has lowered everyone's standards.
I'm sure we'll get more updates than that, but seeing how we still have the original terrible art style, I don't think the game is going to get much more advanced.

That's a very commendable goal. They're pretty expensive and rare though, and if you fuck up a surgery you lose the bionic.
The only downside I can think of is all the hits on the arm regardless of location get applied on the main arm, effectively reducing the hitpoints of the whole limb. But cases where you'd take enough damage to lose it are pretty rare.
