The MoM's real name is Superbia. The name is similar to Vanitas' name being Latin for pride.

Those two are likely connected.

No it wasn't. The reason the dream eaters were able to come out into the real world is because Sora was still sleeping.
When Sora woke up, Riku's dream eaters vanished along with Riku's Dream Eater powers.
