It removes reaction time. If you're already shooting then you'll be throwing bullets down at them before they do. It's only helpful for close range fights because of inaccuracy.

Head if you think you have a good chance at it (stationary, doesn't know you're there, close quarters etc)
Body otherwise (moving often, midrange, close quarters, standing still but also REALLY far away)
Aim for the body when they're moving, aim for the head when they're standing still. Headshots are more important for lower damage weapons like SMGs and pistols or for single shot weapons like Kar98.
