There were some bad diseases that got transferred over to the Old World, but the reason the Old World wasn't as horribly impacted was because people living there had stronger immune systems due to how interconnected they were. Through extensive land and sea traderoutes, China, Western Europe, and everywhere in between had essentially been connected on a microbial level since the days of the old Silk Road. The higher average population density of the Old World also created easier breeding grounds for bacteria, and thus required strong immune systems for any of the people who survived.

A disease is usually more harmful if it is not meant for the host it is in. It wants the host to survive as long as possible to transmit as many germs as possible. So a cold intended for a human is a thing to deal with as a human and can make him chronically Sick. Sick but alive. A cold that is intended for a cow but ends up infecting a human is dangerous as it is too aggressive for the human to survive a long period.
Most diseases that killed the native tribes were originally animal diseases, old world human had been accustomed to. Since native Americans rarely had the extensive contact with animals, old world humans had, they couldnt have been carrying diseases like that. Thus the lack of killing new world diseases

Most plagues come from livestock diseases which transferred to humans

As the NW didn't have many domesticated animals which people lived in proximity with, as well as few densely populated cities, those plagues never got around to transferring to humans
