Afrohistorian here. This one is a really interesting case. It goes back thousands of years after the successful uprising of the African Sheid (shee-id) peoples following the collapse of proto-Korean hegemony in Africa.
One of their less fortunate leaders, Kang Wewuz II, was famously poisoned by his enemies, who put poisonous oils into his royal 40oz. The oils float to the top, so usually they're visible, but because glass was not yet invented yet it was impossible to see the poison sitting in the vessel. As was tradition, Wewuz drank straight from the 40 - and before he knew it, he was dead, and the Sheid Kangdom fell into chaos.
Ever since then, it's been a tradition for descendants of the Sheid to pour out a bit of their alcohol before consumption in order to get rid of poison. Over time it became a superstition about bad spirits, then a symbolic ritual as the history was forgotten. When the white man enslaved Africans and brought them to America, they brought their tradition with them and kept it alive in song and even connected it to the Christian traditions of Heaven and the Afterlife, while in Africa itself the tradition died out due to an inability to actually afford liquor.
The modern tradition, then, survives only in the black descendants of the Sheid Kangz in America, otherwise known as the Niggas.

Pouring out a little bit wouldn't remove the lower density poisonous oils. They'd always float to the top, tipping the container sideways and pouring some out wouldn't exclusively remove the poison, as it'd just float to the top again.

It might do, if you do it carefully, but the effectivity would depend on phase equilibria between the material Sheidians used for the vessel, the drink and the snake oil.
The current tradition is using a more "ceremonial" approach. Just like whites used to hit their glasses together, so the liquids would mix, but for the sake of cleansiness they just touch the glasses now.
