Because you're not seeing the centuries of grime, wear, and horse manure these cities and streets had to deal with to be liveable. Pollution too from the fact that every single living quarter needed a fireplace to survive. They weren't clean places. They never were. They look more solidly built because they were. They needed to be to survive being shit on for centuries. These people barely had an idea of what cleanliness was, most of the time they operated on the humors and miasma theory and thereby coated their cities in shit as a result. Shit on the walls from the chamberpots being dumped, shit on the streets, shit on people's heads. They were all coated in a fine layer of human and various other animal's feces. Nobody cared enough to clean it up because there'd just be more tomorrow. Think about street shitting in India. Imagine an entire city with that idea of sanitation, and you have your typical ancient city (barring rare exceptions like the IVS).

They were fucking disgusting places packed with bandits, disease, and human filth of all sorts. The larger the worse off. All the shit that goes through modern sewers to be treated today ended up in the local waterbody or on the streets. Occasionally a richfag has his private sewer line into the local waterbody or there would be public latrines like the Romans. Helminth infection rates were huge up until the modern era.

no cars and no ability to build a structure much taller than 6 stories. Romans managed to make it to 8/9 at times but without modern sanitation technology and heating its really shitty living at the top story of one of those.

and besides which they wouldn't be comfy at all. If you want an idea of what it would be like, go look at some pictures of 3rd world slums. That is what it is actually like to live without proper sanitation in crowded poorly built small structures.
