You're on the path to diastolic hypertension if that's your regular. Your heart is working against some sort of resistance to the point that it's going through hypertrophy, which sounds good, right? But that means that it's ventricle's size is decreased, and with that, stroke volume goes down, which brings cardiac output down as well. Your heart rate may naturally elevate to compensate, but since your heart is hypertrophied and beating so fast, it has no time to rest for itself, which decreases it's myocardial diastolic perfusion time while simultaneously increasing it's energy consumption. This both increases oxygen demand and decreases oxygen supply. Your body will compensate for this through several metabolic mechanisms, but eventually you may hit a point where these compensatory mechanisms fail; and/or you will suffer an infarction related to some event that increases oxygen demand or reduces supply (ex. hot sauna speeding your HR, exacerbating the problems I already described)

Since your systolic is normal then the afterload your heart is pumping against is probably not vasoconstriction, but may be from excessive fluids related to artificial hormone fluctuations or other substances you consume, or renal damage, which can occur form the previous two things as well.

preworkout would be more likely to cause systolic hypertension, putting excessive strain on arteries especially, making aneurysm risks greatly increase, especially in structurally weak areas such as the circle of willis, or the descending aorta if you're tall. Combine an aneurysm with preworkout, and you're at great risk for a rupture during your workout, and there's not a lot that can be done for you at that point, you'll likely pass out and bleed out inside yourself before you get to the hospital.

Dangers of low blood pressure are related to your heart not effectively delivering enough blood/oxygen to the rest of your body, particularly your kidneys and your brain. However, as you age, your blood pressure naturally tends to lower, and we are actually finding evidence that slight hypertension in your 60's and 70's is a good sign because it means despite your arteries becoming stenosed, your heart is maintaining output and pushing force.

There's also something called Mean Arterial Pressure (MAP) that you have to keep in mind. You find it by taking the systolic blood pressure, plus 2 times the diastolic, and dividing it all by 3. So a blood pressure of 120/70 has a MAP of (120 + (2x70)) / 3 = 86.67. If your MAP is 70+, you are adequately perfused from a clinical standpoint, and for critically ill patients we prefer to keep them above 65 as much as we can. Naturally a healthy person will be much over 70, but this should alter your perception of blood pressure a bit.

For example, a blood pressure of 90/60, while low, still has an adequate MAP (70), which means all organs are receiving enough oxygen, assuming other factors are normal (such as RBC count and hemoglobin.) Often times it is not found until the autopsy is done. Many cardiac condition, such as cardiomyopathy, present no serious symptoms, and may be as simple as exacerbation when you exercise, and who doesn't get that? The person just thinks they're a little out of shape. The problem with this one in particular is that when the heart has to always pump against so much force, as with hypertensive patients, it slowly begins to fail. As it fails to pump blood out, the blood remaining in the ventricle after a heartbeat increases, and it gradually blows up, like a balloon. All of that extra blood, just sitting in the heart, can form a clot, which may be ejected into the systemic circulation at any time. Depending where the clot goes, you could have a stroke or pulmonary emboli, either of which can kill you if you do not get immediate medical attention, which is difficult to do when they also make you lose consciousness. This is why regular check ups are important, just from listening to your heart and getting labs drawn every month or every three months can let any trained specialist know if you're at risk or if your condition is worsening.

What you are describing is likely not cardiomyopathy. Usually the odd beat that you describe is something called a Premature Ventricular Contraction (PVC) and a surprising amount of the population experiences them. Normally, they are not treated unless you are symptomatic, but if you are regularly experiencing orthostatic hypotension it may be worth seeing a doctor. A common cause of PVC is low blood potassium and/or magnesium, often found with malnourished people, but taking your height and weight into consideration it is likely not that. The main danger with a PVC is that it can send you into v-tach (heart attack) which is life threatening. If you ever experience chest pain that is not related to GERD, go to the ER immediately, even if you feel fine afterwards. They will be able to diagnose and prevent future risks.

As for your orthostatic hypotension, that may be caused by a few different things, and you would need to be assessed by somebody in person to get any concrete ideas, but to prevent it, do not stand up from a lying position immediately, but rather go from lying or sitting back, to sitting straight up for a minute or two, and then stand. This may not even be a heart disorder, but may be dehydration or low iron intake (again, given your weight I am inclined to not ascribe to malnourishment.) It can also be caused by things that slow down your vagal response such as antidepressants, marijuana or other ilicit non-stimulatory drugs, opioid narcotics, etc. Almost anything that may make you feel sleepy can cause it.

The basics of why it happens are that when you rise, gravity pulls blood away from your brain. To compensate, your vagal nerve in your carotid arteries (neck) stimulate your heart to beat faster, which ejects more blood volume and increases your blood pressure to compensate. This process normally happens very quick, but anything that delays it can make you either feel like you're about to pass out, or actually do so; because of the lack of blood to your brain for that instant you stand.

For both of these experiences, all I can say is do not rise quickly, make sure you are eating adequate potassium and iron, evaluate any "downers" you might be taking, and see a a doctor in real life.
