At which point does fetish content actively start detracting from the game itself instead of being an element like architecture, fashion, food or other flavor elements?
What can this kind of content bring to a detailed game world where appearance norms and sexual trends are addressed?
Does fetish content inherently detract from the enjoyment a group can get out of roleplaying (assuming that all players volunteered for the game knowing what was the plan), or are people automatically having badwrongfun past a certain point?
Is it inherently wrong or "deviant" to include fetish content in a game played in complete privacy with willing friends?

As long as it's not one of those absurd fetishes like vore or diapers, you can have an undercurrent of eroticism going on the entire campaign.

In essence, the problem with sex in campaigns is not the presence of sex itself, but that DMs do a poor job of using it, players do a poor job of handling it, and both spend way too much time on it when the topic comes up - treat it tastefully, I suppose is what I'm saying, but at the same you don't need to handle it tastefully at all (is crude sexuality and smut not just as valid in stories as tasteful nudity?) so I guess the best advice I can give is this;

I agree with your main points, but I'd like to point out that (at least in DnD) vore is one of the more likely fetishes to arise naturally in game, alongside bondage, GTS/size fetish, mind control/hypnosis, and transformation/genderbending. All of these already have in-game rules, which can't be said for the more vannila fetishes.

My groups can do some pretty lewd stuff but since nobody hangs a big fuss over it it just doesn't matter.

While DnD does have monsters that can swallow you whole, I wouldn't really consider that to be same as having vore fetish stuff. Juts like having characters placed in handcuffs isn't the same as bondage fetish stuff (although in both cases somebody who has the fetish might find the situation arousing). Also, it seems to me that same size vore is a lot more prelevant than getting eaten by a monster vore. If the GM made us fight against Frakass's golden elves I'd assume that it's probably a fetish thing, while if he made us fight a purple worm or another big monster that can swallow a character, I probably wouldn't think anything of it.

False dichotomy. You can have a game as a full ERP yet still keep the RPG side of the game consistent, fun and interesting. I'm in a D&D game where political negotiations with a dragon were concluded by several PCs (Including mine) agreeing to bear the dragons favour in exchange for his favour and assistance. We had full on dragonfuck scenes, but we're also considering the logistics of caring for young dragon whelps and figuring out how to best leverage him to aid our nascent kingdom.

This is one of those things I'll never understand about vote fans. I mean, I can enjoy (soft) monster vore, but something about same size has always seemed really damn strange to me.

But I guess that's just how most fetishes work and dwelling too hard on why they work just isn't a constructive use of time.

As long as the GM isn't the one forcing his fetishes onto the group, and as long as he doesn't let his arousal impact the game, it's completely fine.
The reason that you usually don't want people getting worked up at the table is that it'll impact their roleplaying and cause them to make other players uncomfortable, not that it's automatically bad that someone is aroused. You're trying to protect the whole group's fun, not systematically excise the "wrong" kinds of fun from the game at the expense of people's enjoyment.

This is an entirely arbitrary distinction, though, and one almost entirely based on personal preference.

Sure, you might not appreciate written out sex scenes, but their place in novels shows that a huge number of people do, and it's similar in RPGs. Roleplaying a sex scene isn't just about being gratuitously explicit. Sex is a highly intimate act, and through it you can explore a character in a whole new light. Plus, yes, it can be hot as hell if the people you're with know how to write it properly, but calling it entirely gratuitous is flatly false.

Then again, I should state for context that this is an online game, and we mostly reserve main session time for whole group events like combats or the aforementioned negotiation. The rounds of dragon dicking were done in one on one side sessions with the GM.

The dragon was in human form because one player was squicked out by the alternative. A couple of us are into it, but we're willing to make compromises in order to ensure everyone is having fun.

I really don't think you'd have any problem finding a game like that.
I GM'd an Aberrant game that was basically "play your magical realm without any guilting", and though it broke down about ten sessions in due to a complete mongoloid, it was still a great game and it took me all of one hour to find a full group for it. The group is still ticking, because we kicked out the mong, but we've since moved on to Black Crusade.
All you'd need would be to post that exact same request in the gamefinder thread, and then run it off Roll20 with no issues whatsoever. If anything, reserve the right to boot retards breaking character from the get-go.
