MAD only works because everyone knew that before your nukes would hit their targets, the enemy would launch theirs and there is practically nothing stopping them once launched.
Whoever goes first, wins. It would actually be even worse, because the aggressor would send all of their nukes against the enemy launch facilities. Why bother taking out the command or government or crippling their economy in the first strike if you know they'll wait for your nukes to hit.
