Hmm, maybe just have her be a big old werewolf pack leader, with that more supernatural brand of lycanthropy that results in nigh immortality.
Maybe shes trying to establish her own kingdom for her followers, pushing other species and their countries out of their territory. maybe rousing the wild beast in the land to start terrorizing the countryside.

Her big plan is to eventually get the parts, ingriediants, and arcane texts to create a huge magical ritual to instill a bit of werewolf tainted blood into the bloodstream of every living human within a sizeable area of the county. Trick is that the ritual is complete enough when the party gets there that all that's needed to complete is the letting of ANY blood onto a altar and the party has to decide between slitting their own blood to "safely" finish the ritual, try to destroy the ritual (Hard option), let the ritual complete, diplomacy the werewolf queen into stopping this madness, or let the bard infect the continent with AIDS.

Well they're called the "Alpha", not queen - basically a wolf pack is ruled by a "mating pair" of alphas who spend most of their time stopping the other wolves from boning, and then make the rest of the pack raise their offspring as their own.

It's kinda funny how you can see the beginnings of eusociality in wolves actually, just on that edge leaning towards building hivewolves which WOULD have queens.

You honestly could have the whole Werewolf thing be a secret, and have them be 'attacked' by a few of them out of Lunar Phase whenever she's about. Like, that's her ability she can just Shift whenever she feels and can make others do the same. Make it a generally tough fight and if she keeps showing up that could work out.

Or you could go the whole other route and have it to where she and her merry band of murdering werewolves play musicals as they fight and as long as they're singing (howling) they are almost unbeatable. Their plan which they loudly sing of is to make it that every night is a full moon and every day the shortest day possible.

Why are you guys so worried about the werewolf queen having a sexual past? Like so many of these posts are critical of even the insinuation there is something sexual going on.

What about making her an older werewolf, a little gray in the fur with her litters grown and leading packs of their own. She's the BBEG because the kingdom has been cutting down her people's forests and slaying the werewolves for their magical pelts (invulnerability is one hell of a perk) and she's responded with war of sorts.

She's tough, motherly, regal and absolutely vicious when it comes to her children.

Not a lot of monsters can pass for humans, so:
Have it be the queen of the local constabulary. Recently married into it, but is actually gathering power and protection. Someone in the court discovers her secret and gets away to hire neutral third party (the players) outside of her immediate control. Very tough to kill, would heal very quickly and would be well guarded. How would characters in the court, or the King himself, react to the secret and who can the players trust without getting eviscerated in the night?

The werewolf queen is the younger sister of the crown prince. She was kidnapped and turned by a pack of werewolves, but she quickly rose through the ranks and declared herself "The Wolf Queen". She considers werewolves to be superior, and she wants to share her gift with the kingdom. Her brother is legitimately a shitty ruler and is mostly hated by peasants, so she has plenty of weak farmers volunteering to be transformed so they can join her army and fight to reform the government.

Packs being nuclear family units is as well-known as, say, bird migratory patterns, or the theory of gravity. It's so well known in biology today as to not even merit talking about.

The whole "alpha/beta/omega strict hierarchy" thing only happens in packs of unrelated wolves (which do occur naturally, but only in rare instances, such as where groups of wolves who lost their packs band together for survival reasons).

Normal wolf packs are a Father and Mother and all of their children (including adult children who haven't yet found a mate.) When the children find a mate, they leave and start their own pack.

Actually what's funny is the whole "alpha" theory applies more to monkeys, humans, and like the other user said, prisons. Then that domesticated dogs who get along with humans also follow this more than wolves. Apparently there's almost no infighting in wolf packs, and they certainly have better marital track records than any human.
