I've been looking for a table of consequences. something random as a result of a failed roll. I had some ideas myself for a few physical and mental consequenses such as :
> a mutation
>a serious injury
>physcotic episode
>weakened

Any other ideas? Or if anyone has a table already? I'll post mine when it's finished

No sorry I should explain, it's much like FATE. instead of taking hit points as damage the player can choose to accept a consequence. these are for RP but also allow other players to use to their advantage.

What's actually happening in the game world when you fail a roll for a mundane but difficult task and end up, for example, mutated? It seems a bit too out there.

The consequences arent always a result of a failed roll. It's mostly a drastic way to change the course of the players. Adding tension, obsticals. A mundane fail would only be a mild consequence. A flashbang leaving you with slight damaged eyes (a little more serious) and a grave wound like a lost limb would require the PCs to find medical attention.

A long fuckin' time ago I saw, and neglected to save. Table of events that could stall a Player Character for one session, and the rewards/ penalties that they would rack up during that time. So if you had a player miss a session, you could fill the plot hole of them being missing for a day-week.

Found v2: Legal Boogaloo.
If anyone wants to roll this one, please contribute a table as well or at least keep it to one roll maximum. I've seen this kind of thing kill these threads and this one has been nice so far.

The 40k game Only War might work for Space 'Nam.

You've just got to find a way to take the boredom, fear, and hopeless stupidity and turn it into an actual game. Encourage pettiness. Spend an entire session trying to steal dry socks from Alpha Company. Get dropped into a hot zone, discover that the satchels full of laspacks are actually satchels full of hotshot laspacks... completely incompatible with the weapons you have. Establish a routine of flights or patrols, and then royally fuck it up at random.
