Well i hate your meme, but i actually want to discuss this question. The simple reason, as far as i see it, is because it is literally impossible. It's not that the imperium is just that incompetent, they literally cannot effectively manage their supply chains due to the nature of warp travel. The nature of warp travel is such that you can arrive before you leave, or so late as to have completely missed the reason for your departure. It's random. They have no control over it. It's kinda hard to run a supply chain when its trip through the hell dimension determines when it gets there, not your time table.

Well, it's a little bit of A) All your long distance communications and logistics are required to travel through an unreliable hell dimension.

A little bit of B) It's just too damn big for them to run smoothly.

And a bit of C) 10k years ago a man united half of humanity against the other half and the resulting war almost spelled the doom of our empire and species, so everything is purposefully designed to be inefficient so that can never happen again.

Corruption and dogma mainly. Half of everyone is trying to look out for number one. You have some people like Kriegers and Commisars which are indoctrinated, but very few of them are in actual positions of power. Even the Eccleisarchy is riddled with corruption. The other half of everyone is so deeply rooted in backwards rituals that they can't do anything efficiently or even at all in some cases. The Mechanicus can theoretically innovate, for example, but they have a bunch of contradictory laws and beliefs that essentially make it impossible.

That's facilitated by, contributes to, and works alongside poor communications that prevent any sort of efficient communication or even basic management.

As a real life Elysian, I can say the portrayal of the Guards fucked up supply and logistics is 100% accurate along with their portrayal of officers who have never been beyond a fob.
