I faintly recall you can wield a human arm as a weapon in Vampire the Masquerade: Bloodlines.

You can wield your own arm in Planescape: Torment.

Kerbal Space Program mods sure are getting silly.

Sam's sheath doesn't shoot bullets, it uses blanks and a piston to give him the world fastest Iaido style drawing cut. A few real world weapons use the same principle of a cartridge driving a piston for firing a bullet completely silently without a suppressor.

The Magpul FMG9 is similar in principle.

Probably either covered on the other side like you said, or alternatively the cylinder works like the cylinder on a Nagant, i.e. it gets pressed against the barrel when you shoot, so a minimal amount of gas escapes.
