or
(name) of (bard college or troupe)

though to be honest most people only had one name, only nobles had familial names officially. Most modern last names are either the name of an ancestor of note (owens being son of owen, jameson for son of james, etc) or your family trade (smith, banks, carpenter, loggerhead, etc)
