If the trolley tries to run over 5 people it will kill those 5 but tip over injuring and maiming it's passengers, which will be a lot of pain and annoyance from listening totem scream. But if you run over that one person the trolley is much less likely to have problems and just continue on it's way and the passages will but just slightly traumatized mentally.
