Could be they kept the secret to making more Custodes to themselves, and use it to replenish their numbers whenever one dies or decides to retire for whatever reason. They aren't exactly made in the same way as Marines, but it's likely they keep their technologies in the Imperial Palace so they can make new ones.
Although the mention of them retiring sounds awesome. It'd be neat to see one join a Deathwatch team as a Blackshield. Also neat if they join up with an Inquisitor; a 1k+ year old pinnacle of post-humanism who is too old for this shit teaming up with a firebranded young'un of maybe half a century to flush out Heretics.
