But 3rd trimester? Every little part of the journey suddenly becomes a challenge. She's tired all the time. Walking a mile is genuinely hard. Constant agony as her hips spread to accommodate a baby.
More or less, it becomes a matter of trying to mitigate all of the problems that pregnancy brings, IN ADDITION TO keeping her from taking ANY HP-VALUE of damage.

Sidetracking a bit, but why not have one as the bad guy?
Make the bad guy of the current arc a spymaster, backstabbing conniver character hiding behind a code-name.
Then when the players finally find them reveal her to be a pregnant woman who pleads her belly (aka uses her situation to get out of it/escape).
Obviously it depends on the players, but my group would absolutely fall for it, or at least refuse to kill her.
