There's actually a bunch of novels about Smiling Jack and Cab Buddy. Jack thinks the Cabbie is just a Malk who thinks he's Caine, but there's a lot of is-it-or-isn't-it in them that have been argued to death. The Malk freak-out scene might have just been a Malk being a Malk, because not everything a Malkavian thinks is 100% factual. The PC having a conniption over the Cabbie might have just been more teasing, because if he's not Caine, the Cabbie is a Malk himself.
