The litteral translation (in case you can't Google it) means "slightly undercooked".
"Molto al dente" means "more than slightly undercooked".
Italians are known for undercooking their pasta, and then finishing the cooking process in the sauce.
Finished pasta should be "malvo al dente", or, translated, "cooked very well", or, practically, cooked until the pasta has no bite left to it. It should fall apart slightly when you toss the pasta with the sauce.

Bite it. If you see a little starch in pasta (just a bit, it has to be a tiny circle for spaghetti or a small ring for penne), it's al dente.
