It was done by preplanned timing of each events. Only time they had to re-do that was when Apollo 13 got into trouble.

The most rational explanation is the whole thing was real apart from the descent from lunar orbit. That bit was way too risky and there was no real way to test the technology. So yea other people on earth tracked apollo to the moon and back but the landings were pre filmed on earth and broadcast while all 3 astronauts stayed in lunar orbit.
