It's misguided that games can't have a large amount of content in a small geographical area. Witcher 3 just had some huge cities and their outskirts. Kingdom come will have one small commune iirc, maybe 2? But they'll be almost to scale.
Meanwhile the cities of skyrim have 10-20 houses, they'll be spread with 3-5 meters between them, and they'll still be smaller than that town, but it'll be the capital of a continent and the home of the king
