It's not quite as bad as you're making it seem.

Of the main NPCs, Yuria and Leonhart have placeholder faces. Anri and Horace are hollows, so it makes sense. Eygon is mainly the weird one, but it works for his character.

The way it works is by forcing them not to equip the helmet at all. Any model masks that would hide parts of the face or hair aren't applied.

I think Fromsoft might have fucked something up with the Wolnir fight. There is some shit he can do that can make the fight a lot harder but he rarely ever does it. I didn't even know he could summon skeletons (even the red eyed ones) until my 4th playthrough.

The sword and the damaging mist he throws up on the stage can really fuck you up too.
