The "hand" that he uses to jump at you and grab the ledge is normally wrapped around his chest. He pulls it out when you first steal the hemmed set, and will do so again if you run around by the altar its own in subsequent reloads. It can be finicky sometimes, but he won;t pull the hand out unless you approach the altar, and won;t kill himself unless it's out.
