Elder magic = unicorn magic, hence the elves having access to it -> they tricked the beings at some point, somehow, or betrayed them? In which case, Avallac'h's transporting Ciri to the isle of Mists would be a display of the remnants of this brand of magic that connects life and death (also being signified by the motif of Ouroboros).

It can be not-failed, but you'll lose Dijkstra's support in Kaer Mohren.
The only way
to get 100% is to start Gangs of Novigrad, don't meet with the dwarves, do everything stealthy, and after that meet with Cleaver. You'll get two red crosses, but the quest will be marked as finished.

Then you just have to let Menge torture Triss and ask him about the treasure.

And you get 100% game, with Dijkstra's support, and Triss getting what she deserves.

If you let Menge torture Triss and specifically ask for the treasure, you can ask Dijkstra for help.

Also, Dijkstra apparently recovers the treasure after that.

Depending on the choices in the conversation, he gives you a few men. If you admit you're going for the Church guards, he'll give you "the best men there are who have nothing to do with him."

There's a lot of hidden shit in the game.
For example, there's an unmarked cave in Skellige, where you can find a poet who's in love with a grave hag and there's a very short quest there. Also, on the eastern shore of Ard Skellig, near the ruined forest, there's also a quest.
Some of the quests aren't even in the prima guide.

In Hearts of Stone, when you go to the Herbalist hut when starting the quest with the heist, you can encounter a bloody boar, it leads to a small quest without a journal entry, but it doesn't really conclude anything. If you just fast travel to the Herbalist Hut, you'll never get the encounter.
