Radiant quests for BoS, RR and MM settlements conflict.
You need to play the game more and you would have noticed it happens a lot due to settlements are on ALL the radiant lists.

Depends on wall coverage and what sort of attack it is. Some attacks like ghouls come from a distance away whereas some spawn raiders and super mutants in the middle of a crowd.

So what would be ideal for protecting the settlements? I liked the house in Far Harbor (Visitor Center) that had a North/South exit, so I just blocked off one to force funnel everything through the other. Made defending far easier, but I didn't think I could replicate that for most settlements

I mean it's a pointless gesture, Bethesda exactly knew what they were doing when they required proper legal DMCA takedown requests to get them to act in case of credit theft(that's what this is about, internet fame and possibly donations) but, and that's just as fucked up, outrage and public outcry is the only thing they're going to listen to.
Bethesda could have their platform staffed and moderated 24/7, just like the Nexus does, so they could react quickly to reports but they just don't care. Unless Bethesda themselves are making money off mods they're not going to lift a finger.

They remove that which is reported, they ban the users who repeatedly upload that stuff, hell you still need a Bethesda.net account and a copy of Fallout 4 on the PC to even upload mods. There's a lot of checks, but there's not much you can do to stop Timmy Xbox from uploading whatever he rips from the Nexus without limiting what legitimate mod authors can do as well.

also the elimination of stackable legendary gear with the same enchantment
for instance a full set of power armor with the titan effect makes you functionally invulnerable to attacks while standing still I recently tested this and the only way damage is taken is rads from ghoul attacks
at the end of the day there really isn't any way to make skills work because there aren't a lot of checks to start with, the game wasn't built around having skills
locked specials and level caps should help with the OPness that is achievable
