The first level you can get anytime, the second at 2 hollowing level, third at 6, fourth at 12, last at 16, check your stats page to see your hollowing level after the first level.
Each time you die, you get more hollowing levels, and look more like a strip of bacon.

Okay so about the timeline: the Crystal Sages are the pupils of Logan, and one of them worked with the Abyss Watchers while the other remained in the Grand Archives. So that means either the events between DS1 and DS3 are actually not that distant or those wizards could live forever and the Grand Archives is fucking old, older than the Lothric kingdom itself.
