AFAIK upstab makes spear-wielding NPCs a little better since without it they do almost no damage in close-quarters and also the fact that the AI facehugs everything they want to attack to in melee.

Upstab has been a viable method of fighting for spearmen throughout all of history. It's a versatile weapon that you can use to stab over peoples heads or at their bodies with range.
