Mori designed Izanami and, from what I recall, is responsible for Mai's new design as well. Maifag, that's not an invitation.
And I think Nine may be his handiwork as well. Hi-no-Kagutsuchi certainly seems to be in line with his tastes.
Katou is the mastermind behind Makoto and Platinum. Mori did design Platinum's prototype version, but it was very different from what we have now and far more conservative.

You are correct. While Sumeragi did design Mai originally, Mori was responsible for her new outfit.
It's interesting to compare the final version of the outfit to this early one he had included in Remix Heart. Overall I think it looks significantly better.
