This mainly seems to be with the new Triton/L200 where work applications are more common.

I don't see how that's possible, the 4N15 Tritons haven't even been out for a year. It could be that a few are failing from manufacturing and QC mistakes, that's not as uncommon as you might think. Have you got a link to these reports?
