His entire motivation. Sans explains during the fight why he suddely changed his behavior, and the things he reveals put a whole lot of the other things he does into context.

Really, he's always been a sort of trickster character who seems friendly but has a lot more to him than he lets on. His fight is only the culmination of that.

Alphys. She's the only one in a position to provide the kind of challenge that belongs at the end of the Genocide route.

Imagine the game in which Sans is literally being just a funni pun skeleton. Not stalking you the entire journey, not creeping you out in restaurant, not judging you at the end. No unnerving absence of something and harsh and surprisingly emotional confrontation on your way out to show you how fucked up killing Pap was. No weak, but smart and determined opponent to break power level ascencion combo.
I hope someone will make a patch that will make it happen.
