So when I was patrolling plaguelands I went back to the area you start the game in.

Had to unlock it with a splicer intel key. Ghost started talking like something was going on.
Entire area was full of Thralls. Lead up to a ship that had Thralls and a wizard on it.
Killed everything then left the ship. Ghost started talking again.

You get the Khvostov frame and quest from the chest that spawns on the Fallen Ketch. If you've already gotten the Khvostov quest from dismantling your old Khvostov, then you get absolutely fucking nothing and the game doesn't indicate for a second that that's the case.
