So to do the Outbreak Prime quest, you need to complete activities with a fireteam of a Warlock, a Hunter, and a Titan. I got the quest on my Warlock
The activities are the Nightfall, 3 events in the Plaguelands (Forge probably counts), and 3 heroics or crucible matches.
