If you turn on a monitor and wipe before reaching the next checkpoint, you have to activate it again.
It can be avoided by simply activating that monitor when you're done with Vosiks.

If by really high LL you mean +1 than what I already had each time? Yeah. It was a perfect example of Murphy's Law.

Not sure why they decided to take away any chance of the strike specific legendaries dropping normally when legendary engrams seem to be dropping way less frequently now and the drop rate of keys has to be ludicrously low.
