There's really no canon material on it as far as I am aware, so you can believe whatever you want.
I go with the generic explanation of a 359 degree vision and basic chakra vision, but clearly there are enhanced feats of skill like seeing the chakra points and possibly into the body proper (being able to see the skeleton or muscles, rather than just a black background to the chakra). To take it father into headcanon bullshit areas, one could presume some materials are easier to see through and need no effort, while others might be a lot more difficult.

Good question. The anime always made it seem that the user was focusing on a target area, as it wouldn't be easy or make much sense for one person to have a completely full-round view of their byakugan range (if that makes sense.)
