Some states have mandated that they be removed.
It's considered a fire hazard, once the tank reaches full it can cause excess fuel to splash back at you.

In Canada not all gas stations have those awesome locking levers to keep the nozzle lever depressed so you can fill up hands-free. Its retarded, some stations of the same brand will have it, some wont.
