I dunno what triggers it. Though I being unfair to "official" machine translators, since nobody expected them to translate such trash text in the first place. On the other hand, translation aggregators and other specific soft can produce really good results. I bet it's possible to make specified scenarios for sex scenes that need to be translated once and then 95% of those scenes will be replaced with already translated text automatically. Japanese often make heavy use of onomatopoeia during H-scenes so the machine translator doesn't churn out something very sensible in English.
That was the reason the Haru Uru "translator" gave for leaving the H-scenes in Japanese and I assume it's probably the same for a number of others.
