I Form Shift, Shoulder Tackle boss,Blood For Blood + Internal Release, Demolish, Perfect Balance, Snap Punch, Snap Punch, Twin Snakes, Bootshine, Forbidden Chakra, with GCDs like Elixir Field and Howling Fist in between. I normally hit the mob with Fracture + Touch of Death in between skills, but I'm not sure what the optimal point is for those to maximize damage.

If you're using basically any proper MNK opener, the best time to apply fracture at first is immediately after you refresh DK/Twin Snakes for the first time. So something like this...

This opener doesn't use a potion because I'm a cheap fuck, but it can be adjusted to make room for one. If you use B4B right after the 2nd snap, it should fall off right after Fracture applies, and (assuming you have a "normal" amount of SkS) all of this lines up your DK/Twin Snakes/Demo to be refreshed at around 3 seconds rather than 4 or 5, so you get the most out of them if your latency doesn't cause them to fall off completely right before they reapply. )
