Are the wraiths in Demon all from other players or do some of them generate randomly or are they modified somehow or what? I'm ridiculously surprised running into them on the 5th floor and seeing people manage to have teams of almost entirely modified demons but the game seems to want to contact the server when generating them.

They are either local ghosts or taken from the server.
The modified demons could just be recruited from random spawns.

well there were also some QoL changes, that blue tint, and things like the True Man -> True Kin, the regard system, etc. but all in all there wasn't a noticeable difference, at least from Joppa to Golgotha
