In the first place IIRC Kaito was calling Saihara by his last name instead of his first name until the meeting by the window in chapter 5, and it's quite evident to Saihara and Maki that Kaito is skipping training for some reason so I don't think Kaito would respond badly in that scene unless he realized Ouma might possibly know he's dying and he's not just sick.
