Because the Grail forces servants into classes, so there's no chance of a double up. Look at how Kayneth was disappointed that he summoned Lancer with a fragment of Diaumund's sword in Zero.

Unless is cheating or some rule abuse, the 7 normal classes will be filled first.

They have different eyes and hair color, so same facial detail doesn't affect much.

And Enkidu's appearance was originally Shamhat's, so I don't think it was that important

Sometimes catalysts have connections to more than one servant (like the Round Table shard for example).
e.g. If I summon with that catalyst when there's already a Saber running around, I'd get a servant from a different class instead.

Because the grail and the summoning are an automated system. It gives the command soells and it fills the servant classes container. The master part in this us incredibly minor. As soon as they do anything the Grail can interpret as a resuest for servant to cole forth, it'll answer and pop a servant out.

The catalyst isn't a class selection, it's a link to certain heroic spirits, normally one but sometimes a group. these heroic spirits typically qualify for multiple different classes.

Let's say you use a fragment of Gae Bolg to summon Cu. His preferred class is Lancer and the catalyst has a link to gae bolg, his noble phantasm as a lancer, so it'll try to summon him as a lancer.

But let's say lancer has already been filled. It can't summon him as a lancer no matter what because there is already a lancer. So it'll try berserker. Berserker filled? Caster. Repeat until it's a available class.

Fake assassin is fake assassin. The only assassin summonable by that grail is hassan, but servants can't summon other servants and it kinda shit itself when medea successfully did so, creating a fake servant. This counted as filling the assassin class but not as summoning assassin, so when zouken came along and used fake assassin himself to summon hassan, pop. out comes hassan.
