Yeah, you HAVE to have it in your hand for the buff to be active.
It lasts forever too.
I have base stats for the banner, and the damage is insane with both buffs.
It is 60 Faith.
Base stats for Lothric War Banner.
I put everything else in Att, End, and Vig.
I rely on summons and offensive miracles. I don't kill people with my weapons. I weaken them, but I kill only with miracles or let teammates do it.

l think blessed enchant actually scales better with faith and does more damage on LKGS. lt's actually the weapon that better scales with faith with blessed infusion

Check mugenmonkey.com/darksouls3/weapons and see for yourself. Each point from 30 faith onward gives +10 physical damage
