Is he somehow related to Mettaton?

It's good and you should write more, not just that one specific story (I've seen too many things that were only ever really meant to be oneshots force themselves past their natural lifespan because the masses demanded it and spiral out of control) but in general.

As an unrelated sidenote I'm kinda disappointed how most of the older male chara stuff plays up the androgynous femboy part, it's a nice aesthetic but there were one or two awesome depictions that were fit as fuck and I forgot to save them :(

I remember reading something where Napstablook could see Chara but thought it was too awkward to mention it. Not sure what the context was.

It's a Charrator idea.
Napstablook will respond to the Check text, so the idea is that Napstablook can hear or even see Chara, but as you said, might be too awkward to mention it.
Problem with that is he's not the only character who does that since Tsunderplane does it as well.
So unless Tsunderplane is a Tsundere Ghost who inhabits a Plane, then that part of the theory doesn't make much sense.

Aaron does it too, and he's even less likely to be a ghost.
Napstablook being able to see Chara can still work out without involving Charrator, though.

And Charrator is where the idea originated, but you're right that it doesn't have to involve Charrator.
