It's all about upbringing. My grandfather taught my father who to drove manual, and thus my father taught me how to drive manual. Some people can learn to drive manual of their parents never taught them, but most people don't go this route. Automatic is standard in America so most people don't learn it unless their parents teach them.

Dumbass manual question here: On a relatively steep hill is it more fuel efficient to ascend at a lower gear or a higher one? I figure the gearing disadvantage means I have to use more fuel to maintain my speed than it is to maintain it a lower gear, even if I'm using a slightly higher RPM
