I don't have everything completely figured out yet.

In terms of who's swapped with who, what roles everyone has, Asriel and Chara died and were brought back through Flowey and Frisk, like in canon but the circumstances are swapped. But because of the nature of the new circumstances of Asriel's rebirth, I still need to think things through more and figure everything out. Psychologically, I have him figured out for the most part, with reasons for why he would go Genocide and canon already applies why he'd go True Pacifist (and thus between them both, lies Neutral), but whether he's a Flower or Boss Monster, and why everyone would want to fight him, is what I need to figure out.

Yeah. I don't normally read fanfics at all but that story and the story before it (part two I believe) are some of the few I have read and enjoyed. I have a hard time picturing Muffet as a parental figure to Frisk or smoking a cigar but other than that I really liked how silly they are, as if they were in a cartoon. It's vastly different from what I expect fanfics to be.

Not really... He was just a ghost that became a robot. I haven't really seen any actual sign of 'trans' in there. That seems mostly like something the fans came up with upon learning his story.

Napstablook and "Napstablook's cousin" are consistently referred to with "they" pronouns, even when that's really clunky. It seems like ghosts are amorphous sexless blobs of ectoplasm, which kind of makes sense.
But Mettaton is male.

But "Napstablook's Cousin" was referred to as 'they' under a circumstance of "this person is someone that is not really known or present nowadays and everyone only has a vague awareness of their existence).

Sure, but it's something that's very unusual about Napstablook. As far as I remember Frisk, Chara, Monster Kid and Napstablook are the only fleshed out characters that are consistently referred to with gender-neutral pronouns, and ghosts are known to be very different from other monsters. It's plausible that it's just true for all ghosts.
I don't think it's canon, but I do think canon hints at it.
It's the odd one out in because the other ones genuinely don't fit with the game. Then again, I don't know what the stereotypical fanon trans Mettaton is like. I try to stay away from that sort of thing.

So I've been thinking more about my sequel plot idea thing about where Gaster causes anomalies by piecing himself back together, and True Pacifist and Soulless Pacifist timelines meaning and everyone getting lost in the void and stuff... and apart of that is Chara from post-Genocide somehow gaining some manner of 'control' over the 6 Souls and sending them after Frisk and the others from True Pacifist timeline, and that's mostly what I've been thinking about lately. A subplot I wanted to deal with is Monster Kid's faith in Undyne being restored.

So what I came up with is, at the start of the story, Undyne rescues Monster Kid from an anomaly thing, and that's what brings the Kid into the story. So from them on, MKid is being watched over by Undyne while lost in the void and stuff. More stuff happens where Undyne saves Mkid's life, but they still don't see Undyne as their hero again yet. But eventually, the kid encounters the SOUL of Integrity, and they are out for blood/dust. Integrity attacks Mkid and nearly kills them, but Undyne rushes in and saves them, goes Undying against Integrity and Frisk probably helps out, and in the end Integrity is defeated, and Monster Kid's faith in Undyne is restored up to 1000%, but Undyne is melty and stuff, and Alphys sees this and in desparation to save Undyne's life she needs to find a way to extract that extra DT from her SOUL.

How does this sound?
