There have been 3. The first two it was a list of characters you get a random 5* from that list. The third one, they had so many 5*'s that they split them all up into different classes, you pick a class and get a random 5* servant from that class. The one exception was Assassin, since Jack was the only 5* servant at the time, so they had Jack + Extra gacha, with Jack and Jeanne.

We will probably get the original which gatcha which gives you a ten roll and guarantees you will get one of the following:
