I think is basically character specific side quests and interactions for them. Custom characters only have the origins like soldier, mystic etc etc.

Is a frontline spear fighter decent? I used to play one in DOS1 where you just stand in the front and try to get attack of opportunities.

It looks like you will not be very tanky this time around though since spear is finesse based and I believe heavy armor has a strength requirement? Dodge tanking seems to favor dual wield too due to parry master.

They take place in the same universe, but as far as the plot goes they aren't related. I don't think anyone was saying that they weren't related. Just that you wouldn't miss much by not playing the previous games.
